Mainteance Loan Request Form 2011/2012

When applying I was accessed for the Maintenance Loan but didn't request it as I didn't think I'd need it. I've changed my mind. Where can I find the 2011/2012 Maintenance Loan Request Form? I can only find the form for 2010/2011.

You can just apply on the website. Login, go to your application, "Make Changes to your 2011/12 application", "Change your loan amount".
