I'm unsure as to what to do regarding student finance, little help?

Basically I still have the statement that was sent to me in the post which looks like it needs to be filled in at the bottom and I have the payment forms; Payment Conformation Form, Card Payment Instruction and Stating Order Mandate.

I feel like I should have sorted this by now but I have not been spoken to about it by anyone and am a little worried.

Would appreciate any help.
