Ethics form for OCR coursework..help !

I'm really fed up of my old school
I dropped out of sixth form in october
+am continuing my a level in psychology at home with a tutor
+still take the exams at my own school
In regards to coursework, it has to be sent through the school[centre]
But I cannot for the life of me get in contact with my old psych teacher
I've phoned, I've left messages, I've emailed
+she's hardly ever in so it's not like I can go in and find her.

OCR originally told me my tutor could mark it
Then just have everything sent off through school
They're now telling me I need my psych teacher to mark it
+that I need the ethics form completed before I can carry out my coursework.
I have no idea how to get this all sorted
If I can't get in contact with the psych teacher
I'm screwed.
My first aim is to get the ethics form sent off
But I'm fearing it's too late.

Anyone have any ideas of what I could do ?
Or by chance a copy of the ethics form that needs to be sent off ?
If I fail my exams its all my old schools fault.
I'm so annoyed. :mad:
