Well i'd contact UCAS and ask

But for general guidance, as far as they do it with UK A-level grades... they (usually) give a single predicted grade (I.e. A or B or C... etc). Im assuming it would be the same seeing as any conditional offers through UCAS are likely to specify specific grades which you need to achieve (although not all offers are made in this way)

So its most likely they just need to put a single grade down - although its a good idea to contact UCAS/Unis yourself and ask for guidance
It's up to your teachers really - if they think there's a chance of you getting a 5 but don't want to outright predict a 5 then 4/5 is ok (lots of UK teachers predict A/B )
