Being a referee for someone's job application

Hello everyone,

I have been asked to be a referee by a former colleague who I used to work with, in a takeaway almost a year ago. After that, I worked for a while in the Accounts department of a Company but because of Covid-19 have been made redundant. I do not want to disappoint him as he requested me for a reference. His potential employer has emailed me a form to fill which is asking about my job details. If I write Unemployed in there, would it be pointless to fill out the form for him? Or it does not matter as they only care about the past job?

It is asking 'Job Title of Referee', 'Name of Organisation', 'In what capacity do you know the Applicant', 'How long have you known the Applicant'. So the first two, would it be for my current or should I just write what title I had in the previous one? and the name of that Organisation?
