What even is that?

An official exam paper? Examiners won't likely mark you down if it didn't affect the correct answer but it may annoy them
If it's a school paper, your teachers would definitely be annoyed lol (depends on how nice they are)

It will probably just be ignored.. However, if you write or draw anything obscene or offensive on a formal exam (such as a real GCSE) then the paper will be sent back to the exam centre for them to decide any disciplinary action and the paper will be given 0 marks. I've seen this happen a couple of times.
