Questionaire

For my new assignment at college, we have to do our research project. I have a short survey that I need people to fill in. The survey is about how people feel about themselves and whether they have considered surgery or if they have had surgery. it would be greatly appreciated if people could fill it out so I can get as many responses for my results. Please answer every question even if you haven't had surgery there are still options for you's.

However, I do have some advice for future surveys.

Since you are collecting data from the public, even though this is for a college assignment then a consent sheet specifying:
1] The purpose of your project
2] Why you are collecting the data
3] How you will utilise our data in your project
4] How you will secure our data
5] How you will comply with the GDPR. I.E. How long you are planning on storing the data, If the data isn't anonymised then what are your procedures if a person wants their data deleted, etc.
This should make your participants more comfortable towards completing your questionnaires and also covers you legally

Additionally, I noticed that there are some overlapping ages which could confuse other participants such as:
[20-25 / 25-30]
[35-40 / 40-45 / 45-50].
In later surveys then it would be better to format ages as:
18-19
20-24
25-29
30-34 etc

Additionally, I noticed a few spelling mistakes in the survey that confused me what you wanted the answer to be:
I.E, [It cost _ & I had it done _]
Please proofread later surveys to prevent such issues
