Struggling with this question

You observe a colleague being asked by their supervisor to open and unpack a delivery, using the approved safety cutter, then put the items on to a trolley and take them to the store. You notice your colleague starts opening boxes, but the cutter breaks so he throws it to the bin, from a distance, and cheers when it goes in. He carries on opening boxes using a knife.

Unbelievably, another team member then gives him a ride on a cage before he loads it up and takes it to the storage area. Which four of the employee responsibilities with equipment rules have been broken here?

1. Never interfere with equipment to make alterations or cause
damage
2. Take reasonable care of yourself and others who may be affected by what you do
3. Wear appropriate personal protective equipment e.g. safety glasses
4. Cooperate with others working with you
5. Use work equipment safely and its intended purpose

2,3,4 + 5 seem correct here. I would guess 1 isn’t one of the 4, as it doesn’t mention anything specific he has done to alter or cause damage to equipment. Hope this helps.

Definitely breaks all except 1. Throwing objects, getting lifts etc can be very dangerous and is not careful. Using the wrong item for the job afterwards and not reaching out to get an approved item shows poor co-operation and also means they aren't using the appropriate safety items.
