Motivating yourself + sticking to a plan

Finding it really tough recently to plan out work until super close to the deadline, where I just work and work and work a few days to get it done.

I really would like to try spread it out over, for example, the month to be able to deal with the workload more consistently.

I try to remind myself with things of where I want to be in a year, or two's time, but it's still quite tough for me to break out my habits.

Could people share what they've done to help this?
