homophobia is one of these made up words that doesn't really make any sense, another such word is 'Islamophobia'

But actually, what if your complaint was more focused on the homophobia and discrimination and the grief aspect, how would you make known to them you want to persue the complaint on discrimination grounds (Clause 12) and let the family decide as to whether they wish to persue the complaint on Clause 5 (grief) grounds...

To be honest: it would be a waste of your time and a waste of the PCC's time. The article singles out no individuals for discrimination and therefore Clause 12 is not relevant. Comments like those made in the article are ones that the PCC dare not consider in its remit (nor should it).
