Declining firm choice uni

Does anyone know how it works if you decide you no longer want to go to your firm choice uni, after having made the offer?..
Presumably if you defer you'll have to start from scratch and lose all your offers.
Would it be a simple case of ringing up UCAS/uni or do you have to get agreement from the uni to withdraw your place?
Any info on this would be helpful

You say, "if you decide you no longer want to go to your firm choice uni". In this scenario, what do you want to do instead?

Swap your firm and your insurance choice? Select one of the (up to) three courses which you turned down when you chose your firm and insurance? Change the course you want to study, but at the same university you originally firmed? Retain the course and university, but simply change the year of entry by one year (i.e. defer)? Not attend university at all in this cycle, and decide later what you want to do?
