Organisation name on UCAS reference form

Hi, so I'm sending off my UCAS application today and need to fill out my reference form. I was essentially homeschooled and the reference was my tutor for a brief time.
The relationship I put is tutor, but I'm unsure what to put under organisation. Should I put his current job title/place of work? Or should I put the tutoring company I found him on (which would be Tutorful)?
