You can't actually do this. Your 5th choice has to be a non 'early deadline course' so not another Med, Vet or Dent choice. And even if you were able to do this, because of your Medicine Personal Statement, it would be obvious to your 5th choice Uni that you dont actually want to be a dentist and therefore you would be rejected.

Look instead at courses like Biological Sciences, Biomedical Sciences, Chemistry, Pharmacy, Pharmacology etc.
Many - not all - Unis will allow '5th choice' Med applicants to submit a 2nd PS direct to that Uni, but you need to double check this with the Uni before you submit your UCAS application.

Useful reading :
Biological and Biomedical Sciences - Study - University of Liverpool
Undergraduate | School of Cellular and Molecular Medicine | University of Bristol
Chemistry and chemical engineering | University of Southampton
Choosing between Pharmacy and Pharmacology (bath.ac.uk)

You can apply for a mix of medicine and dentistry with all 5 choices, but it needs very careful assessment, as many unis read PS to make sure people are committed to the subject they are being assessed for and having a PS for medicine and applying for dentistry (or vice versa) is likely to result in a rejection
