Do I need to take the IELTS?

I am a 11th grade WBBSE student educated in an English medium school. From 1st to 8th grade, at school, Bengali was my first language, English second and Hindi third, from 9th to 12th English was my first language and Bengali second, and English is also the medium of instruction for all other subjects from Kindergarten to 12th . But my mother tongue is Bengali.
In UCAS should I select 'yes' or 'no' when asked whether English is my first language? And is it compulsory to take the IELTS exam?.... since most university where I wish to apply accepts English as a medium of instruction
