It would make no senses to only being able to make an insurance choice. If you don't meet the conditions for your firm offer but are accepted for your insurance choice, you're obliged to take up the place unless either they release you or you withdraw from UCAS completely, just as with your firm choice. Having only an insurance offer wouldn't mean that you could choose to turn it down and enter clearing without their agreement.

But what if you only have one choice and then make it firm. You will be obliged to commit to it, as in there's no way of dropping out once you've made it your firm.

You can withdraw completely from UCAS for the year at any stage which will effectively release you, but you'll be unable to go through Clearing if you do so. You can also contact the university directly and they may be willing to release you on request, in which case you can still enter Clearing.
