You go to the web site of the university you want to transfer to and search for "transfer" or "transfer in". It'll either explain the process or tell you that they don't allow transfers. Note that the answer may vary by course.
Note also they they will almost certainly want you to meet the entry requirements of their course. (So your A levels, BTECs etc. must meet their year one entry requirements, even though you're looking to transfer into year two.)
