It’s up to each school to develop their methodology for assessing the students and forming a prediction, but it would be a combination of seeing your year 12 performance (in A-level maths) plus how you have performed in class/school before your predicted grades are placed on UCAS.

If you are taking Maths A level now there will be several weeks of FMaths teaching before the end of term. Grades might well be tweaked at the start of Year 13 too.
