It's too late to apply for Fall '09 and you already seem to have offers in the UK...so why?
How about for fall '10 - say for a PHD programme?

You can apply to US uni's regular decision program until March 15th.

You need to visit www.collegeboard.com and look for university's and their application dates.

You also need to register to take SAT's. You've got your work cut out.

Other than taking the GRE tests, you don't need to do much in advance to apply to PhD programs. A typical application consists of:

1) Application (easy to complete)
2) Writing sample (may or may not apply for math)
3) Personal statement (why you want a PhD in math, why that university, etc.)
4) Recommendation letters
5) GRE scores (general test *and* math subject test)
6) Transcript

CMU has a very good guide to the graduate admissions process. It's geared for those in computer science but applies to most fields.

Hi. I am thinking to apply to a university in America. Pls help me:

1. How can i apply?
2. Universities which are good in maths if you know
3. Will be ok with the A Levels?

Don't bother because you won't be considered for anything decent. And you won't be okay with just A levels. You would have needed SATs, SAT IIs for anything good, college essays, teacher recs, and all that other stuff.

Btw, why didn't you try warwick, probably better than imperial for math and not difficult to get into. And it feels pretty close to a lot of US campuses.
