How lenient are LSE?

Have any students from LSE ever been let in for an Economics or Economics and Economic History course without A Level Maths?

I don't actually know the answer but it's probably no one. I don't know how you could be applying for courses that are so quantitative and not have been comfortable enough with maths to take it for an A-level. Additionally they say it is a required subject, so they would be well within their right to reject you before they even read the personal statement, unless you have done something (maths based) extraordinary outside of school and didn't pick maths because your school wouldn't let you.
