CAS requested LRN From

Hi, this week I have requested the CAS (due to the UCAS embargo I was unable to do it until this week) and the CAS Team has emailed me this: "We are unable to make an assessment on your CAS request until we have received copies of the following documents: LRN Form". Do you know what that document is? I can't find any information on the internet. Please help.

Unfortunately I have not heard of this 'LRN form' before, the only thing I found on the UCAS webpage for LRN is for the Learning Resource Network.
I would highly recommend contacting the International or CAS team to get the right clarification for this matter as soon as possible.
