Expand the brackets. Differentiate each term, some of them require implicit differentiation. Then factor out dy/dx and solve for it.

4x + xsquared.y + (2xy+ysquared)dy/dx = 0
as only x terms are differentiated
And all other nomials have to follow prudct rule chain rule and quotient rule
