Welsh Bac Questionnaire

Hi All,

My 15 year old son has to do a Welsh Bac project and needs to include some research.
The topic is how the NHS compares to the Health Care System in the United States.
If you have received any treatment in the US can you please take a couple of minutes to complete the questionnaire?
Most of the questions are tick or cross questions.

Could you use one of those questionnaire sites so I can tick boxes etc rather than completing and uploading a Word document. You'll probably get a lot more results that way.

ps. I live in the US and have seen many doctors for many things. Although I have lived in the UK and seen doctors there as well. I can probably give you more info if you want, send me a PM.
