Welfare and mental health support whilst doing year 13 at sixth form?

I had some issues last academic year, mainly getting worse for the entire year before getting a lot of help from february and march onwards, up to and including the end of the school year. I now receive similar help to keep my going. Just wondering if anyone has had a member of staff from welfare at their sixth from help them for the entire year and if so what was it like and did it help for the long term too? I would also appreciate DMs on this and also what it is like from a teacher's perspective and how this help is given and what can be done from a teacher's point of view in order to help a student excel at what they do, academically and mentally? For a bit more of a reference, I do 4 A Levels, I believe I can do it but at times I need a bit more support to make sure I keep going and continue to do better rather than just cruise the entire year, and make sure I'm doing ok mentally too
