Re-applying for a PGCE next year?

I'm currently a final year undergrad and have an offer for PGCE for this year.

Due to health reasons, I think I may need to defer my final exams until the supplementary exam period (August). I then wouldn't get my final results until September and wouldn't graduate until December/January.

Obviously, one of the conditions of my offer is that I graduate, and it states that all offer conditions must be met by 31st August. I've done some research and it seems that if you want to defer your entry, you still need to meet the offer conditions by this date. Although, it appears that not many course providers allow deferral of a PGCE offer.

So basically what I'm asking is if I end up deferring my exams, would I then have to reapply for a PGCE for 2022 entry? Is it worth contacting my training provider to ask if I would be able to defer even if I won't have met the offer conditions by 31st August?

Thanks.
