How to structure Yr13 revision

I was wondering how would be best to study for A-levels through year 13. I am really aiming for top grades A/A* and was able to achieve AAA during my end of year 12 mocks with revision only in my study periods at school.
However, going into year 13, I really want to put more work in and try to get the top grades.
How is everyone structuring their revision for their subjects?
I'm looking to put in 4ish hours per day on top of my schoolwork and study periods.
