For access to university? Almost never. For most courses at most universities, they'd ignore the fourth A level anyway, when making offer decisions. So all it does is increase your workload and/or reduce the amount of time you can dedicate to the other three A levels. This therefore risks your grades in the first three A levels being compromised by the fourth. AAA is a better grade profile than AABB (for university entry).
