That's a bit difficult, because some chairmen take small pay out of their companies, like six figure salaries, but are billionaires already.

No, it didn't make sense, I was trying to ignore it.

I admit $100m is too steep but the mean pay of the top 15 US CEOs is $56,469,000 per annum.

It' very difficult, there is no clear route you just have to work your way up companies and get on a board of one (a challenge in itself) and then try to become CEO of a company. Remember, there are only 15 CEOs in the world earning that type of money.
