I would say characterisation is part of the structure in terms of it helps develop the plot and structure of the novel.

Form is something relating to what type of literature. So if it's a novel, the form of that novel would be of an epistolary form if it were written in a diary, letter or extracts from news paper articles. For example, Stoker's Dracula is an epistolary form novel.

Structure is how a piece of literature is developed through plot, theme and characterisation.

I would say characterization, could be wrong tho
