It's for Argos and it has no specific instructions on it. I don't know whether application forms are usually in block capitals as it mostly just one word answers except from your grades and employment history.
Yes do it in block capitals and in black pen - that way they can't pick you up on anything later.

I did mine in block capitals - but only for short answers on the form. When it gives you a box to write in, I usually use lower case too.
