For veterinary medicine/science you have to fill out work experience forms that require the contact details for your placements. Some vet schools also require references to either be submitted online or to be brought with you if you are selected to attend an interview.

I'm not sure about optometry. Each university website should state if they have work experience requirements and how they want proof of any work experience that has been completed (if work experience is required).
