Moving Uni after foundation year (year 0)

Hello.
I'm currently studying Health and Social Care in London Metropolitan University. I'm doing 4 year course and it is my year 0 (foundation year).

I applied for Mental Health Nursing through UCAS, I had successfull interview and I got an conditional offer from University of Cumbria.

I have few questions.
1) Do I have to resign from my current uni? When it is the best time to do it? At the end of the academic year? Earlier?
2) How can I provide details and grades from moduls I'm currently studying? I already passed 4 modules. 4 more to go.

I'm a bit lost, could anybody help me please?
If your offer is conditional on you passing modules at LMU then
a) ask Cumbria what evidence they want - email them and find out
b) work out when that info would be available - email your course leader and ask, but without mentioning Cumbria
c) dont resign from LMU, don't do anything, until you are certain you have met the offer, and been accepted by Cumbria.
d) to resign, you simply email LMUs Academic Registry and tell them you will not be progressing beyond the Foundation year.
e) don't forget to also tell SF at that point that you are switching Unis..
