just the F-word. Only one and it is 3 weeks away and this is without revision but still...I wanted a bit of a break. The fact that the paper is sort of synoptic for the last two years aswell annoys me.

Well done on the race for life!

Thanks  I've just made myself a cooked breakie of garlic mushrooms, spinach, eggs, potato bread and smoked bacon! NOMNOMNOM.

Thats a pretty impressive score for 3 weeks prior to the exams, it's crazy how much you can learn in three weeks so I think you'll be fine! Oh and don't worry about it being synoptic, the exams I've just finished were for my last three years and some of the stuff I 'revised' I was like 'when the hell did we do that?!'

question, so in the context of complications of MI the magic book says:

'persistant RVF presents with high neck veins and hepatic congestion. In the acute stage in sevear cases, Swan Ganz monitoring may show low left sided filling pressures with high right sided pressures. Cardiac output may be improved my plasma expansion in carefully regulated amounts, rather than diuretic therapy which may make the situation worse.'

Why? RHF would have low pulmonary perfusion and low return to the left side, is it saying sod the right heart and improve the systemic perfusion? surely expanding the plasma volume is not good for the failing heart, excluding hypovolemia? Or is it the potassium effects from diuresis??

Help please

You're right in once sense - careless plasma expansion would bugger the heart.

But what you're missing is the point that the problem becomes one of hypovolaemia - what will the heart start doing if you diuretic the crap out of them and they lose loads of water ---- the heart will beat like a mofo and cardiac output will actually WORSEN - increase the volume slightly and the converse happens - the heart will beat slower and actually help out the failure and be more efficient = better cardiac output.

Ah yes. My dirty pint was just absinthe and bolt. I downed it instantly. There was a look of horror on many peoples faces. I was fine apart from the energy drink hyperactivity...well fine considering the alcohol intake...

Yeah I am probably stressing for little reason, I did just skim the paper...
Jealous on the breakfast but I had nutella so ha!

...I've lived on nutella and toast for the last few days cos I've been too lazy to make proper dinner :colondollar:

Beat like a mofo! I love it, someone who finally explains physiology in the same terms as me!

I have eaten well this week Sea bass on tuesday and steak with mushroom sauce on wednesday neight of which I had to cook. Then I made pork meatballs with a tomato sauce and spaghetti on thurday and curry on friday. Yesterday was home made burgers. I don't have the stamina to do a roast today though...

I like cooking

That's really impressive! I love cooking as well but just don't have the energy (or money) for it these days.

Normally I'd make Irish beef stew for my boyfriend coming home from his army training but I think tonight he'll have to make do with some chicken fajitas

I want to be RAMC but according to the website if I apply now I wouldn't even be guaranteed getting an F1 job :frown:

Yeh I was thinking of going down that route but I'm asthmatic and you need to be asymptomatic for four years before they consider you, so another three years then :s-smilie:

Ever consider just doing your F1 job through the NHS then joining the RAMC? Is that even possible? (Sorry I'm a bit ignorant about all this stuff, Glasgow likes to only let you know stuff at the last possible moment, most often the night before )

Fajitas are nice though.

I am funny with stew, I only let people I trust cook it for me...too much can be hidden in a stew...

Beef
bacon
onions
parsnips
Garlic
Carrots
Red wine
Beef stock

Simple as that! Served with crusty bread and butter. Love your paranoia though!

Ah well basically if you don't join as a student you generally have to then join as a specialist e.g. registrar in something. You'll still get the golden hello (£50k) but yeah means you gotta wait and will be older when you're doing basic training lol.

Damnit shouldn't have been so lazy about it when I was in OTC
Free BBQ and beer today. Literally cannot wait.

I imagine you fit in rather well at Imperial. Sea bass I ask you... did you round it off with some brandy and a couple of Cuba's finest?

I know, I wish I knew more clearly what my career path was rather than 'uh I suppose the NHS, sure they'll work it out for me??'

Golden hello sounds pretty sweet though...

Actual lol.

Although that said - I may or may not be in enemy territory right now.

I'm enjoying the Imperial library :ninja:

Well tbh if I do what I really want to do - it's not even available in the army as they don't tend to do many transplants  But then I'd do trauma type stuff instead...
we'll see, blatantly end up an endocrinologist and nothing I've planned :p:

At least you have an idea what you want to do, some days I'm not convinced I want to do medicine at all! I applied for an intercalated degree in cardiovascular studies, I don't know whether that'll light the cardio fire under me or extinguish it...

Worse case scenario is that we all end up dermatologists
