Agree with AEH - most likely answer is extremely over-aggressive fluid resuscitation. Somebody may have extremely over-zealous with metolazone or an equally potent diuretic. Probably a combination of the two.

Another possibility is a spurious lab result or a drip arm sample etc..

I didn't take the first sample which came back at 110 so I assumed it was a mistake/from the drip arm so I repeated it from the other arm and it came back at 105. Pt has had some fluids but only a couple of L a day as they are NBM.

Pt also seems have haematemesis.

Was on diuretics but had been for long time.

It has the whole team quite bamboozled.

It has the whole team quite bamboozled.


Haematemesis - is she a boozer or a secret boozer?

If she is has underlying cirrhosis - it is likely her negative feedback system that decreases ADH secretion in response to hyponatraemia is buggered (technical term :p: ). That would contribute to it.

But hey if your team is stumped - it is probably something a bit bizarre. :yep:

Good question - she is a stroke pt newly started on asprin so probably has an ulcer - she is for a scope tomorrow (if she makes it to tomorrow :frown: )

You will have to keep us informed as to the diagnosis.

Waits for the geek to point out that the PPI cover for her aspirin alongside the likely ACE inhibitor (especially if a boozer) she had for secondary prophylaxis may have been contributory factors. But I'm not a geek. :o:

Consistently dropping or sudden plunge?

Sample taken downstream of a drip? Overzealous fluids? Vomiting? Diuretics?

Can't salicylate toxicity do that? i.e haematemesis and hyponatraemia?

Very unlikely for therapeutic dose aspirin given PR to result in toxicity and even less likely to result in haematemesis.
