Declaration form not being sent?

Hello. My first declaration form was sent to the wrong address, so I printed it, signed it and sent it but SF said they didn't get it... So they sent me another but It didn't come... Although it did say in correspondence it was sent to my current address. Then ANOTHER one was sent to my address but NOTHING has come through the post :confused:

What should I do? If I request another will it ever arrive? If I print it and post it again, will they receive it? Do I have to track the envelope to make sure SF get the declaration? :frown:

Hi Mark, It's been over 2 weeks. When I called last week the guy told me he hasn't received anything...

2 declaration forms arrived, one yesterday and one today! They were dated for the 19th and 20th - I posted one of the declarations today and will keep the other as backup. PLEASE make sure you tell the person who deals with post at your offices not to lose it! :frown:
