I received wrong grade and sent it to UCAS what should I do ?

I have recently finished my UCAS application and sent it last week and my old comprehensive school just sent out GCSEs certificates from like 2 years ago which I got today, this whole time I thought I had passed my English Language with a (C) and that's what I had on my sheet with exam results which I applied with to college but the certificate that just came through states I was awarded a (D) so now I'm stressing out because you clearly need a minimum (C) in English Lang to do any course at uni and I don't know what to do? I'm studying A level English Language and Literature and I doubt what grade I get for it will be able to substitute for the GCSE.
Here's what I'd do.

1. Phone your old school- you need to be sure of your grade.
2. Phone UCAS and get it changed if it is wrong.

The unis will need certificate evidence of your grades before you start the course so a wrong grade would be a problem!

Thanks for the help! But I doubt I will get in the course by this point .
