UCAS Reference Organization

So, I am applying through UCAS as a student from the United States. I would like my former engineering teacher to be my reference as I am looking at engineering programs, but he left my school this year and is now working at a US university.
Should I put my high school for his organization since that's where he worked with me or the university that he works at now?
Also, how does anyone know how predicted grades work coming from the US? I am doing the IB diploma, but I don't have any predicted scores. Someone recommended that I ask all of my teachers for a predicted score and then give those to my reference, but I'm unsure what to do.
Lastly, what should I put down for my reference's address and phone number? Should that be their personal information or the information of their institution?

I dont know all the answer but:

1. Put down their professional address/phone number so like their office.
2. Put down where he works currently as his organisation

3. as for predicted grades do what you already said, and ask your reference to mention them
