Where is this form? (postgraduate)

I switched my master's course at the beginning of the year and SFE have sent a letter asking for information about the new course (see attached).

It says to download a copy of a form www.gov.uk/postgraduateloan. This URL redirects to www.gov.uk/funding-for-postgraduate-study and I can't see any forms.

Anyone else here had the same letter and figured out what to do? Or please could SFE help out?

(By the way, I did already get the first payment installment - it came through in September as scheduled, before the course switch happened. I just got this letter a couple of weeks ago, immediately once the switch was processed by SFE).

(I called them - turns out my application was flagged by mistake and there's not actually anything I need to do. Oh well!)

My payments never were unblocked, so I called again and turns out I do have to resubmit the application myself - was flagged by my univerisity so can't be fixed on SFE's end. Have to fill it out on paper and post to SFE (not freepost )
