It really depends on the university. If you're worried you might fail second year you'd probably be best just talking to your academic supervisor, but most universities should have advice about transferring/failing second year somewhere on their website.

Do you want to transfer to a different university or repeat your second year?
To transfer you need to submit a UCAS application asap including a reference and details of your current university study.

Are you planning to not hand in assignments?
If you passed first year and some of your second year modules then other universities will consider you for entry to restart from yr 2.
Speak to your tutor about getting a reference today.
