How do some patients lose so much weight

I know it’s probably depression but it’s scary how they can lose half their body size is less than 3months when they where genuinely overweight
People losing weight very rapidly is not usually a good sign...

Often losing lots of weight over a very short space of time is a sign of crash dieting or a serious illness.

I'm not sure what the context of this question is, but when I went through really bad depression it effected my appetite.
I lost circa 35-40kg in 3 months.
I'm healthy now.

In fact when I first started olanzapine (bipolar), I piled on weight and ballooned to 85kg. But I'm a decent weight now at 74kg.
Pretty high chance it's due to cancer
