Student Finance Based on Household Income Form

I'm currently filling out the ''Applying for Student Finance based on Household Income form" to amend my current finance.

I'm an individual student that's been supporting myself more than 3 years, where do I provide evidence for this or is this at a later stage?

Whats the best evidence to provide? I know you can now log on to the Gov.uk website to get past employment history, would this be sufficient?
