Name different from name on Birth Certificate

I've just submitted my student finance application (undergraduate) and have been asked to upload my birth certificate as proof of identity. However, my surname was changed a few years ago so isn't the same as the one on my birth certificate. I have a change of name deed poll letter - should I just upload this along with my birth certificate?
