Doctoral Loan - pdf application

Started a PhD in 2021 but cannot now apply for a doctoral loan online - rang your contact centre and was told I'd get sent a copy of the application form to fill/return. Have received this today but the pdf is corrupted. Would it be possible to get a working version sent over without my having to ring your helpline again which takes ages?
