The postcode for your declaration form depends on whether you applied online or in paper. Use the postcode on your particular form (PO Box 210 is for paper applications only).

If you're sending evidence with it then that should be fine but read the instructions - if the postcode/PO Box on YOUR evidence instructions on your account is different to the postcode for your declaration then you'll be better off sending them in two separate envelopes to the two different departments.
