How bad does your acne have to be?

I know a lot of posts get made about acne as its such a common annoying thing but i was wondering how bad your acne has to be to want & need to see a specialist?

I have been going to my GP for around 4 years now due to acne on my face, back, chest, sometimes i get them on my arms but my GPs keep telling me different things and my last one said it was my testosterone levels without even doing tests or looking at me and i feel i'm just being messed around, would also like to point out that i have been on three antibiotic courses in a row with not even a few days apart and currently using differin gel only. My acne has calmed down a lot as i have had to cut out ALL junk food, exercise, certain food, eat loads of fruit which are all triggers and even if i go gym just once during the week my acne flares up massively and soon as i eat a pizza for example my acne flares and i'm in a position where i'm being expected to live a certain way just to keep my acne under control, my question comes down to basically how bad does your acne need to be to see a specialist without being laughed at and told its not so bad?

Should i see a specialist and should i go nhs or private? i just want my acne/scars gone but i feel like if i go in like this il be laughed at but to me its more than just a few spots like they see and i know as soon as i stop following this acne life avoiding gym and junk food my acne will break out.

I'm absolutely not qualified to answer any of those questions because I don't have acne, but it definitely sounds bad enough for you to see a specialist. The antibiotic courses are also insane - they destroy everything that's good and I still don't get why doctors in the UK hand out antibiotics this easily.

(Also, I just looked it up and it even says online that you should avoid antibiotics treatments for acne) - or for most other things, in my opinion
That many antibiotics courses and still having acne tells me whatever the cause it’s not p acne’s or anything to do with inflammation acne and clearly whatever is causing it antibiotics can’t attack and clear where as accutane targets about 4 things If I remember correctly.

I’m not a specialist so don’t take any of what I say as advice lol.

Hi,

I started to get acne at 12 and it got really bad as a teenager, I tried everything - pills from the doctors, expensive soaps, creams from other countries and facials and though some of that stuff calmed my skin down a bit it was only ever short term.

My acne started clearing up slightly at 18 but recently, in my early 20s is when I have had clear skin for the first time in since being 11. My skin has become so much better, this started when I went to a different doctor at my doctor surgery. He told me antibiotics wouldn't be sustainable as you can become immune and therefore it wont have any long-term effects in treating and managing my acne so he prescribed me something and I can say I now have clear skin, my skin is now smooth and I do not have acne anymore, I may occasionally get a tiny spot but as you probably know a tiny spot is really nothing!

My doctor prescribed me 'Epiduo 0.1%/2.5% gel (adapalene/benzoyl peroxide) gel' This gel is great, it cost the prescription price and after a week there was noticeable difference in my skin and after a month I no longer had painful and angry looking acne, I had clear skin!

After my skin was clear I kept on using this gel nightly and then a couple times a week to ensure nothing comes back. It works wonders and has been the only thing that has worked for me long-term. I highly recommend speaking to your doctor about this cream! When using this cream my skin became very sensitive but my skin adapted and after 2 weeks it was no longer sensitive. It is really strong stuff so avoid your eye area, once i accidentally rubbed it near the corner of my eyes and they became so sore. If you do get this cream make sure you have clean skin before hand, wash your face and put this cream on the areas you have acne. Then put moisturiser on the areas you do not have acne at night. In the morning make sure you moisturise your skin after washing it.

Please be careful if you look elsewhere many companies take advantage of people with problem skin and rip you off - it has happened to me so much.

I hope this helps you and you can get this cream as I think it will work wonders for you! Just call your doctors and ask for an appointment (mine are doing them over the phone)

In this case you should see a specialist yes. They shouldn't deny you because you have had problems with acne for over 4/5 years and just because it isn't as bad now doesn't mean it is not bad. You have to really state your case as doctors only have their notes to go on - say how this is affecting your mental health, how it has been years and how you have tried so much and you would now like to be refereed. If you state the facts of everything you have tried and say you want to be referred to a specialist then they should listen. If they don't then you may have to seek things out privately.

Have you been to a salon? dermatologist? had facials?

Yeah it is expensive! I think mine was heavily down to genetics for it to be as bad as it was in my teens and then other factors as I got older.

I think they only prescribe if it's quite serious, you must make it clear to the doctor that it's interferring your life and mental health. My acne was all over my face, chest and back and the doctor at first gave me a weak antibiotic which didn;t do anything so she then gave me isotretinin

Personally yes. My acne is completley gone and the red scars I had all over my face have pretty much faded.

I'm no doctor but if you've tried everything else then isotretinoin is probably going to be your best bet as a last resort

If you go through the NHS how long does it take to actually get on the drug?
It took maybe a month or two as you have to get blood tests and the referral to a dermatologist, and after that it's usually a 6 month treatment

Is it true you have to go there and be looked over by a few people and looked at up close?
I had my GP look at me who then referred me to a dermatologist who examined me and agreed to give me isotretinoin. You do have to get regular blood tests as well

Does isotretinoin help with scars?
It did for me, I had a lot of red marks all over my face and they've faded so they really aren't noticable at all anymore

For me I havent had a spot since I finished my treatment, and it supposed to prevent them for the future as well. although my brother who also took isotretinoin did flare up again a couple years later and he had to do the treatment again but I think it's uncommon to get acne after treatment

Tbh given what you've said it might be worth directly asking for a dermatology referral - you've got nothing to lose if you ask.

Thanks for the replies guys :smile: i will also add that every time i see my GP he's never once mentioned referral or anything to suggest he's going to pass me on to a specialist and every time i have been back to see him he's put me on another antibiotic, he's the GP who put me on x2 antibiotics in a row after already seeing i had previously took lymecycline twice, my friend said he's not wasting time with GPs and NHS and gone private and just got accutane just from asking for it within 3 days...he went yesterday and they said sure do a blood test same day and then in 2 days we'll have accutane ready to pickup...but its cost him £2,000 which is a lot of money for treating acne which is why i want to go NHS but his acne isn't as bad as me and he's tried no other treatments first so i find it so annoying that if you NHS route you can get easily denied and it takes so long when private it takes not even a week...

Can someone explain how i could get to see a GP or get a referral during times like this and if i was to be told yes to a referral would it be delayed months due to corona still? also what do they check for in blood tests? i don't do drugs or drink.

You just ask directly - state your case. And yes, it likely will be delayed. Most things are at the minute. They check for various things and various medications can mess with your body so they need to have a base line to compare it to. It's not for alcohol or drugs - they'd tell you if it was. It's just formality.

I would probably say if the Gp knows you he might just do voice call and hear you saying it’s not worked and refer you or he may ask to see pictures of a video of you first but I can’t imagine him finding something else to give you as GP are very limited with things to provide especially with how much you’ve tried already.

They may do, especially for something like this. It might be worth taking pictures as well. If they say no, then wait until you can see someone in person I'd say. You'll be waiting a while regardless, so waiting a little extra time if this doctor is useless isn't gonna really make a whole lot of difference.

Ask your pharmacist about a pre paid certificate. That should help.

Okay, i'm going to phone them in morning and hopefully ask to see/talk to my GP instead of the others there as no offence but they're so useless, half the stuff they've told me i already knew from google or they straight up don't know anything, i'll see how it goes in the morning, thanks!.

That's unfortunate - I imagine it's hard for GPs to know about everything though lol. I hope you are able to. At my GP surgery you can request to speak to specific people (I only see one person there tbh bc the others get stressed at me).

Good luck with dermatology - I'm under dermatology for a different reason but they are super knowledgeable about everything skin related and incredibly kind/understanding, so hopefully it'll go well for you. If I can help at all just drop a PM. :h:

I'm unsure. Usually it's just a case of putting your point across (like you did in your OP). I've not really had to force them to give me referrals, usually my GP or whoever just refers me anyway? But then I'm not exactly an easy patient to deal with due to a genetic condition I have, so usually my GP/other specialist refers to those who are more knowledgeable to manage whatever is going on at that time. Dermatology was a joint referral with immunology/allergy for me, that's the only reason I occasionally see dermatologists! All you can do is advocate for yourself, explain what you've tried previously, what you're currently doing and what is/isn't helping, that you don't think another round of xyz antibiotic will help etc., and then ask can you be referred.
